CANCELLATION POLICY
For new patient evalutions and physical exams a 45-60 minute time is allotted for the visit. If you need to reschedule your visit for any reason please give 48 hours notice. THERE IS A $75 CHARGE FOR A MISSED APPOINTMENT OF THIS LENGTH AND YOUR INSURANCE WILL NOT COVER THIS FEE.
For regular follow-up appointments please give 24 hours notice if you need to cancel and reschedule if at all possible.
INFORMATION FOR COMPLETE PHYSICALS AND NEW PATIENT EVALUATIONS
1. You will receive a 5 page new patient health questionnaire (or a 1 page yearly update questionnaire) and a registration worksheet by mail prior to your appointment. Please complete this at home and bring the forms to your appointment. The new patient questionnaire requires about 30 min. to complete.
2. Be sure to bring a COMPLETE LIST OF ALL YOUR MEDICATIONS AND DOSAGES or bring in your bottles.
3. We will need a copy of your current insurance card and also to collect any COPAYS or DEDUCTIBLES which are due at the time of the visit. It is your responsibility to obtain information about your health insurance benefits. We do accept Visa and Mastercard for your convenience.
4. Please refrain from wearing BODY LOTION OR PERFUMES to any visit to our office. During a physical it may be necessary to do an EKG and the leads will not adhere properly when lotion has been used. In addition Mrs. Luria and many of are patients are allergic to certain scented body lotions and perfumes, even when used sparingly. THANK YOU. WE APPRECIATE YOUR COOPERATION.
